What is an IFHC?

What is an Integrated Family Health Centre? 

First, it is not just for families. As an IFHC we offer health care regardless of your personal or social situation. 

So really, we’re a Health Centre for everyone. 

Being an Integrated Health Centre means that we work closely with other health services to ensure that the care you require for your particular health need is provided by the best person or service, as close as possible to where you live and as soon as possible.   

Some of these health services used to be in the Hospital, and now they’re based with us, in our building, so their care is available in the community.   
Here are some examples.

  • Our Health of the Older Person team is an integrated team of Kauri and MidCentral Health staff who are based at Kauri HealthCare to focus on the support of our more frail older patients.  
  • Our Child Asthma/Eczema Clinic is run by expert MidCentral nurses and available at Kauri HealthCare to support our patients.
  • Our Kauri Long-Term Condition specialist nurses meet with MidCentral specialists and discuss the care of patients with complex diabetes, cardiac and respiratory conditions.
  • A PHO Clinical Pharmacist works with us to support patients with medication issues
Integration is a work in progress. For a long time just getting into our new building has taken a lot of our time. Now we’re there we plan to expand our integrated health care. We are already talking with other Hospital and community services about ways to integrate, so that we will be able to ensure you get the best available service closer to where you are in the community.
